1. Introduction
Welcome to the website of Wawasan KVL Occuhealth Sdn. Bhd. ("we", "us", "our"). By accessing and using this website and our occupational health services, you agree to comply with and be bound by the following terms and conditions of use.
2. Services
Wawasan KVL Clinics provides occupational health services including but not limited to: DOSH medical surveillance, OGUK medical examinations, marine medical examinations, pre-employment medical assessments, drug and alcohol testing, audiometry, spirometry, corporate health screening, FOMEMA examinations, and vaccination services.
All medical services are conducted by qualified healthcare professionals in accordance with applicable Malaysian regulations and international standards.
3. Appointments and Bookings
Appointments can be booked via phone, WhatsApp, or our online booking form. We aim to respond to all enquiries within 2 hours during business hours (Monday to Friday, 8AM to 5PM). Walk-in patients are welcome subject to availability.
We reserve the right to reschedule or cancel appointments where necessary due to unforeseen circumstances. In such cases, we will make every reasonable effort to notify you in advance and offer alternative arrangements.
4. Fees and Payment
Service fees are quoted at the time of booking or upon enquiry. Payment is due at the time of service unless alternative arrangements have been made for corporate accounts. We reserve the right to modify our fee schedule with reasonable notice.
5. Medical Reports and Certificates
Medical reports and certificates are issued in accordance with the standards required by the relevant regulatory bodies (DOSH, OGUK, Jabatan Laut Malaysia, JPJ). Processing times vary by service type and will be communicated at the time of examination.
We maintain the accuracy of all medical reports to the best of our professional ability. Medical reports are confidential documents and will be released only to authorised parties in accordance with our privacy policy.
6. Patient Responsibilities
Patients and clients are responsible for:
- Providing accurate and complete health information
- Bringing all required documentation to appointments
- Arriving on time for scheduled appointments
- Informing us of any cancellations at least 24 hours in advance
- Complying with all safety protocols at our premises
7. Limitation of Liability
While we exercise professional care in the delivery of all services, we shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services or this website. Our liability shall be limited to the extent permitted by applicable Malaysian law.
8. Intellectual Property
